With the end of 2020, the global epidemic has not come to naught. On the contrary, the level of infections in comparison with the peak of the first wave has seriously jumped. WHO predicts a moral global catastrophe due to uneven receipt of COVID-19 vaccines by poor and wealthy countries. Russia was one of the first to start a vaccination program against a new coronavirus infection: everyone can get vaccinated at the expense of budget funds. Nevertheless, the attitude of the population of our country and Nizhny Novgorod residents, in particular, to the topic of vaccination is still wary.
As part of a zoom conference, the news agency "In the city of N" tried to get answers from specialists to the most common questions of residents of the region about vaccination against coronavirus.
One of the participants in the event was the Minister of Health of the Nizhny Novgorod Region David Melik-Guseinov.
David Valerievich, how do you assess the epidemiological situation in Nizhny Novgorod at the moment?
Now there are no borders between the Nizhny Novgorod region and Nizhny Novgorod in this regard. The regional center accepts patients from all over the region. For example, a patient from Pochinok may end up in city hospital 5. There are 7,775 beds in 72 institutions: we counted each one, and we see in the system whether it is busy or free. Employment is about 75%, another 25% is a reserve. Some more beds are in constant rotation: 400-450 people are discharged per day and the same one goes to bed. There are beds where patients are based on social indications: for example, there is nowhere to isolate themselves (living in hostels, members of large families). They, of course, should not be treated in a hospital, but as long as there is a reserve fund, we can use it in this way.

For the second week, the daily number of newly diagnosed patients does not rise above 500, but also does not fall below 450. How to perceive such stability?
We reached the very plateau we dreamed of. Every day we put the same number of patients in hospitals and discharge them. In terms of morbidity and detection rates, we have stable numbers. So far, the situation is improving somewhat, but we are waiting for the offensive on January 25-26. These numbers should show how the New Year holidays went in the region (whether we observed security measures, whether the distancing was organized properly in our personal space), because the virus has an incubation period. On these numbers, the X hour will fall, by which it is possible to assess the dynamics of the epidemiological situation.
At the moment, more than five thousand people have been vaccinated in the region, more than 30 thousand have applied for vaccination. When will the region receive the promised 151 thousand doses of Sputnik V vaccines?
Just spoke to an authorized exclusive supplier today. You need to vaccinate twice in a time interval of 14 and 21 days. The shipment is due to arrive in early February, but it has just become known that 52,000 doses of the vaccine will arrive in the region by next Thursday (January 28 - ed.).
David Valerievich, name all the options available for registering residents of Nizhny Novgorod for vaccination?
Indeed, residents of Nizhny Novgorod have a unique opportunity to use several channels to register for vaccinations, but it is better not to duplicate them, registering here and there. After all, a medical organization that received information from different sources is a "bottleneck".
You can use the portal of State Services. All medical organizations where the vaccination process has been launched are connected to the portal and see those who have signed up.
The second method is communication channel 122. This is an emergency telephone, but you can still call, although it is recommended to use other recording methods.
There is also a website of the Ministry of Health of the Nizhny Novgorod Region, where you can find a simple form to fill out, where you can also select the nearest point for vaccination.
You can call the registry of the medical organization or come there even without a call and sign up.
How quickly after the creation of the application the Nizhny Novgorod citizen will be called and invited for vaccination?
They will definitely call as soon as they form a homogeneous group of five people, so that in this composition they go through all the stages of vaccination and be monitored for 15-30 minutes after. The entire vaccination process takes less than an hour.
Also, the invitation depends on the receipt of the vaccine in the clinic.
In addition, there are difficulties with the formation of the group: in the evening all five people are appointed, and in the morning two of them do not come. We are still at the very beginning and are improving the mechanism.
Let me remind you that before any vaccination, residents of Nizhny Novgorod should consult a therapist. It is better to leave all clinical questions at the mercy of doctors. For example, I was recently vaccinated and wanted to swim in the ice hole at Epiphany. The question arose, is it possible? Doctor Ivan Romanov (chief physician of the hospital 30 - ed.) Said: "Don't, you were vaccinated a week ago." I didn't plunge, I didn't jump into the hole, although I really wanted to, but in the end I just took a bath.
How do you feel after vaccination?
Okay, I feel fine, I have no temperature. I did not take sick leave and worked without interruptions on weekends.
How long does the process of one application take (the first stage of vaccination)?
The vaccination should be within an hour, including the waiting time after the injection itself. Medical organizations are trying to adapt their spaces to accomplish this task. During January we will work out the cherished hour, we want to become the first in Russia who will succeed. We are striving for this picture, and I gave such settings to the chief doctors on a video selector.
How many vaccination sites are currently in the region, what is the total need for them, and how quickly can such a site be deployed?
Now we already have 58 points (data as of January 20 - editor's note), new ones are opening every day.
It is important that the item is technically prepared, and this requires a refrigerator that maintains a temperature of -18 ° C. This is a specialized box and you can't just buy it on the market. Yesterday such a point was opened in Chkalovsk, by February 5 we expect that there will already be 70 points throughout the Nizhny Novgorod region. The demand for them is growing.
We offered private organizations to take the vaccine free of charge and vaccinate Nizhny Novgorod residents. Let me emphasize: the drug itself in commercial clinics will be free of charge, while the support service may be paid.
We plan that we will be at the peak of the vaccination campaign in late April - early May, and by that time 85–90 points will be operating in the region.
The carrying capacity of such a point in a seven-hour shift is 100-140 people.
I don’t know how the work in private clinics will be organized. Someone might want to launch a charity vaccination story. But we will definitely donate the vaccine for free. Now we are collecting applications from private traders, I cannot yet voice the names of the companies.
What are the requirements for a private clinic to be able to vaccinate residents of Nizhny Novgorod against coronavirus?
The clinic must have a treatment room, and workers must be trained in the use of the Sputnik V vaccine. The facility must have a vaccinated register so that personal information is entered into the federal register. It is important that a private healthcare organization is licensed to vaccinate.
How should the preparation for vaccination be transported and stored, how long is its shelf life?
The cold chain is built from the manufacturer to the specific vaccination point. It is not interrupted for an hour or a few minutes, it is closed throughout the entire logistics. The temperature should not exceed -18 ° C.The shelf life of those parties that go into civilian circulation is six months. In the future, it can be increased up to a year, like the flu vaccine.
Have there been any cases in the region when a vaccinated person got sick?
Yes, there were such cases. Several people have contracted coronavirus after the first application. They were not admitted to the second. According to data a week ago, we are talking about six people.
What should be done to those who feel bad after the first application? Is there medical control of the vaccinated in the Nizhny Novgorod region?
If any of the symptoms indicated on the checklist occur, the patient should contact the organization where he received the vaccine. Here the connection should be from the side of the patients. We are trying to reach out to the representatives of those risk groups who are now being vaccinated. We do this through doctors or social workers.
Will the vaccinated residents of Nizhny Novgorod receive any preferences or concessions? For example, the ability to not wear masks?
Good question, but we will keep the intrigue for now because we are in the process of developing such privileges. They will be, but what exactly - we will inform you within a few days.

The lifting of restrictions depends on the level of morbidity and on the workload of the bed fund, and not on the number of vaccinated. Based on these data, the coordination headquarters makes a decision. If the bed fund is overloaded, we will get a difficult social situation when people simply will not be provided with medical assistance.
What can you say to critics of the Russian Sputnik V vaccine?
They write a lot, including me, about the quality of the vaccine. These questions are heard from those who are pondering, sitting on the couch, and the information voiced by them has no basis. Some believe they know more than virologists. We faced Ebola 10 years ago. Even then I said that the Russian virological school is the strongest in the world. In the USSR, it developed, being built into the military-industrial complex. Those laboratories and those bases that we have can quickly respond to any threat. It is no coincidence that Russia was the first to react to Ebola, and Russian serum is now used in Africa. Other countries have not come close to solving this problem. It's the same with the coronavirus. Yes, we received this vaccine very quickly, but this does not mean that it was prepared for 2-3 days, this is the experience of the world's leading laboratories located in Russia accumulated for decades. Therefore, as a person who worked in the pharmaceutical industry, I have no questions about the quality of the vaccine.
Deputy Prime Minister of the Russian Federation Tatyana Golikova said that Russians will be able to choose a vaccine against coronavirus already at the beginning of 2021. So far, only Sputnik V is being used in the Nizhny Novgorod region. When will we have a choice?
Now in the Nizhny Novgorod region only Sputnik V is used as a monovaccine. Two other vaccines (EpiVacCorona and KoviVak - ed.) Are undergoing post-registration studies. It is not enough just to register a vaccine, it is still necessary to conduct research in the post-registration period on a wider population of people. As soon as the drugs pass the funnel of these stages, the volumes will be assigned immediately, including for the Nizhny Novgorod region. For EpiVacCorona, such additional studies will continue for another two to one and a half months. In some regions, for example in Tatarstan, it is used in test mode.
Nizhny Novgorod region will not take part in the test "EpiVacCorona"?
Not. We will use it only after a full cycle of research.
Is it possible to predict whether the coronavirus vaccination will become regular, like the flu?
I communicate with the developers, my colleagues and I are synchronizing our vision of this issue.Many are in favor of crossing flu and coronavirus vaccines in the future and integrating this into everyday medical practice. This will make the coronavirus vaccine only an integral part of the influenza vaccine we recommend every year. I think there will be one vaccine in the future. We will also be vaccinated during the rise of the epidemiological season - from September to the end of October.
